Notice of Privacy Practices
Purpose of this Notice
This Notice of Privacy Practices describes how One Mental Health LLC (“One Mental Health”, “we”) may use and disclose your protected health information (PHI) and your rights regarding that information under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We are required by law to maintain the privacy of PHI, provide this notice, and abide by the practices outlined here.
Our Duties
Maintain the privacy and security of your PHI and notify you following a breach affecting your information.
Provide you with a copy of this Notice and follow the terms currently in effect.
Limit uses and disclosures to the minimum necessary to accomplish the intended purpose.
How We May Use and Disclose Your PHI
Treatment: share information with other health care providers involved in your care, such as primary care clinicians, therapists, or pharmacies.
Payment: submit claims to your insurer, verify eligibility, and obtain payment for services rendered.
Health Care Operations: conduct quality assessments, licensing, audits, and training to improve services.
Required by Law: respond to court orders, public health reporting, or other legal mandates.
Serious Threats: disclose information to prevent or lessen a serious threat to health or safety.
Other Disclosures: when required for workers’ compensation, law enforcement, coroners, or organ donation processes, as permitted by HIPAA.
All other uses or disclosures will occur only with your written authorization, which you may revoke in writing except when we have already relied on it.
Special Considerations
Telehealth: we verify your identity and location at each session and use HIPAA-compliant platforms. Despite safeguards, electronic communications present inherent risks.
Minors: parents or guardians may access a minor’s PHI unless limited by law or the provider determines that releasing information could endanger the minor.
Your Rights
Access: request to inspect or obtain copies of your records. We will provide them in paper or electronic form within 30 days, subject to reasonable fees.
Amendment: request corrections to your records if you believe information is inaccurate or incomplete. We may deny a request with a written explanation.
Accounting of Disclosures: request a list of certain disclosures made in the previous six years.
Restrictions: request limits on how we use or share your information. We are not required to agree, but if we do, we will comply except in emergencies.
Confidential Communications: request that we contact you at alternate addresses or numbers. We will accommodate reasonable requests.
Paper Copy: request a paper copy of this Notice at any time.
Complaints: file a complaint if you believe your privacy rights have been violated. We will not retaliate.
How to Exercise Your Rights or File a Complaint

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your care.
Changes to This Notice
We may revise this Notice as laws or practices change. The most current version will be available on our website and in our office. Changes apply to all PHI we maintain, including information created before the change.
Effective Date
Effective January 1, 2025.
